The easiest way to get it onto your iPad is likely to be, to download the zip file in the link above, unzip it, and email the .layout file to yourself.

The follow these instructions:

“Open this email on your iOS device using the built-in Mail app. Then, press and hold the attachment at the bottom of this message to open the layout in the Custom Keypad app.”
